InstructionsIn the blank to the left, enter in the UMBC ID of the student you are responding to. This is the first part of their UMBC email address. Do not attempt to access the form by entering the student name. Though this will lead you to the next screen, an error message will appear at the end of the submission process. The form screen should come up already partially filled out - with the student’s name, email, project title, etc. If it does not, you might want to ask if the student has already submitted his/her forms. This is necessary to ensure a successful student-mentor linkage in the database. In the blanks left on the faculty form, you should only need to fill in your rank/job title, department, phone number, your “high/low ranked” assessment, and your personal comments. Then check off the last two boxes indicating your willingness to be a faculty advisor and that you have proofread the student’s abstract. All of the blanks/boxes should be filled in at this point, except one (whether you have an ‘other institutional affiliation’). Click Submit. Another screen will appear with the phrase, “Thank you for your submission.” This signifies a successful submission. If you have left out a mandatory field, a reminder message will appear, sending you back to complete the form.
